On or before April 30, 2012, all applicants must submit the following requirements to DepEd Scholarship Committee under the supervision of Dr. Nerissa L. Losaria, Chief, Staff Development Division, Human Resource Development Service, 2nd Floor, Dona Teodora Alonso Bldg., DepEd Complex, Pasig City with telephone nos.: (02) 633-7237 and (02) 638-9638:
- Form 138/137 with a general average of 82.5% in their 4th year high school report card;
- Birth Certificate (Photocopy of NSO issued);
- Certificate of Good Moral Character;
- 2 x 2 and 1 x 1 pictures (2 copies each); and
- Accomplished Application Form.
To avail of the following scholarship grants, successful examinees who obtain the required scores will be notified by the DepEd Scholarship Committee:

The expenses during the testing specifically the traveling expenses for DepEd examiners and function room rentals shall be charged to HRTD funds subject to existing accounting rules and regulations. Other incidental expenses shall be borne by the MBC. Travel expenses of examinees are charged to their personal accounts.
