Valid ID's Needed For Passport Requirement

Whenever you go outside the Philippines, you need a passport. It is a Primary National Identification that serves as your proof of identity. And it is very important and required for certain types of transactions. When you apply for a Passport, there are valid id's you need to get or secure.
1. NSO Birth Certificate (Original copy) or Birth Certificate issued by the Local Civil Registrar authenticated by NSO.

If you're a passport applicant who has no birth records, and is born in or after 1950 (January 1, 1950 or after, you should apply for a delayed registration of birth at the Local Civil Registry office at the place of your birth. After successful registration, apply or request for NSO Birth Certificate. Bring the also the following supporting documents
  • Form 137
  • Voter's Registration record (example: Voter's ID)
  • Baptismal Certificate with dry seal
  • Office on Muslim Affairs (OMA) Certificate with photo and dry seal for Muslim applicants
If the applicant is born before 1950 and below (December 31, 1949 and below), he/she must get the following documents:
  • Certificate of Non-availability of Record from NSO
  • Notarized Joint Birth Affidavit of Two Disinterested Persons
  • Any public document/s with correct full name, date and place of birth (example, Baptismal Certificate with readable dry seal or NCMF / Office on Muslim Affairs (OMA) Certificate with photo and dry seal for Muslim applicants
2.  NSO Marriage Contract (for Married Women) in Security Paper or Marriage Contract issued by the Local Civil Registrar and authenticated by NSO. When entries in NSO Marriage Contract are blurred or unreadable, it is required to request for a Transcribed Marriage Contract from the Local Civil Registrar.

If married abroad, report of marriage authenticated by NSO is required.

3.  Other valid ids required (at least 1 of the following):
  • Driver’s License
  • Digitized SSS ID
  • PRC ID
  • GSIS E-card
  • IBP ID
  • Digitized BIR ID
  • OWWA ID
  • Senior Citizen’s ID
  • Alumni ID
  • Old College ID
  • Old Employment IDs
4.  Supporting IDs required (at least 3 of any of the following):
  • Voter’s ID
  • Barangay Clearance
  • Police Clearance
  • NBI Clearance
  • Land Title
  • Seaman’s Book
  • Elementary or High School Form 137 or Transcript of Records with readable dry seal
  • Government Service Record
  • SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
  • Income Tax Return
  • Voter’s Certification, List of Voters and Voter’s Registration Record (for newly issued VRR, please attach receipt)
  • School Yearbook
  • Community Tax Certificate
