I. RATIONALE

Partnership with the private sector has been one of the strategies of PhilHealth to attain its mandate of enrolling all Filipinos to the National Health Insurance Program (NHIP). This strategy is in response to the Aquino Health Agenda on Universal Health Care otherwise known as Kalusugan Pangkalahatan. The enrollees/members of the NHIP are provided convenience and more options on the venues for enrollment and payment of premium contributions to NHIP tapping private sector. The members, including its qualified dependents, may opt to avail of the I-PhilHealthy Card being issued by PhilHealth's Private Enrollment Partners (PEPs).

II.    FEATURES OF THE I-PhilHealthy CARD

The I-PhilHealthy Card serves as a privilege and discount card to enrollees and members of the National Health Insurance Program (NHIP) who opt to avail of the card, at cost, through PhilHealth's PEPs.
  1. The member's i-PhilHealthy Card shall contain in the PhilHealth Identification Number (PEN), Full Name, Date of Birth and Sex. The member shall affix his/her signature at the back portion of the I-PhilHealth Card.
  2. The cost of the card per member is Fifty Pesos (P50.00). The card's cost For the First two (2) qualified dependents of the member shall be at Four Pesos (P4.00) each. The card's cost for each additional qualified dependent/s, shall be at Fourteen Pesos (14.00).
  3. Interested enrollees and members of the NHIP shall proceed to the nearest PhilHealth PEP centers in participating branches of SM Retail Inc. and Bayad Center to avail of the I-PhilHealthy Card. Companies, organized groups or communities who are interested to conduct on-site enrollment or hatch printing of their employees' I-PhilHealthy Cards may submit a letter of request to the Chairman of the PPP Committee.
  4. New enrollees of the NHIP shall be required to properly accomplish and submit the PhilHealth Member Registration Form (PMRF) with the attached documentary requirements to the said PhilHealth PEP center.
  5. Existing members of the NHIP shall present their PhilHealth Identification Card (PIC) or Member Data Record (MDR) for the generation of the I-PhilHealthy Card.
  6. The enrollees and members of NHIP shall pay the appropriate cost of the I-PhilHealthy Cards requested. The PhilHealth PEP center shall advise the enrollee on the schedule of release of his/her I-PhilHealthy Card.

III.    AVAILMENT OF I-PhilHealthy ENTITLEMENTS

All I-PhilHealthy Card holders, upon presentation of the card, may avail of the various privileges and discounts when purchasing drugs, medicine and medical supplies, and services from any of the participating merchants as listed in Annex A.

IV.    AVAILMENT OF NHIP BENEFITS

All I-PhilHealthy Card holders shall be entitled to NHIP benefits, subject to existing policies and procedures on benefit entitlement.

V.    EFFECTIVITY

This Circular shall take effect upon publication in a newspaper of general circulation.
