Company Name - the name of company, business, or biller that you want to pay for.
Date - the current date upon payment
Institution Code - can be left blank
Product Code - can be left blank
Subscriber's Account No. - your account number as a subscriber to the company biller
Subscriber's Name - your name as a subscriber
Cash Payment Breakdown - you can leave this blank especially if your money is not the exact amount of the payment you're going to pay and let the BDO employee do the work.
